"To hold out" is not quite the same as "to hold it".
You are holding out on me. = You are not giving me everything (information, money, cookies, the whole truth, etc.).
I held out as long as I could. = I was strong for as long as possible. I resisted for as long as possible.
I can't hold it anymore = This often is used to mean "I have to go to the bathroom NOW!
(However, it doesn't indicate by itself that the speaker needs to urinate, and that was the heart of the question.)
